Most metabolic research targets hormone receptors. 5-Amino-1MQ targets an enzyme. Specifically, it inhibits NNMT — nicotinamide N-methyltransferase — a cytoplasmic enzyme that methylates nicotinamide using S-adenosylmethionine as the methyl donor. That reaction consumes two things simultaneously: nicotinamide (a NAD+ precursor) and SAM (the universal methyl donor for DNA and histone methylation). Inhibit NNMT and both accumulate. NNMT expression is elevated in obese white adipose tissue. Preclinical work — including research from the Brenner lab — suggests NNMT inhibition shifts adipocyte metabolism toward increased energy expenditure. Not by directly stimulating thermogenesis, but by altering the metabolic program of the cell. The NAD+ increase feeds into SIRT1, which downstream affects mitochondrial biogenesis and fatty acid oxidation. The SAM conservation effect also alters methylation-dependent gene regulation.
The honest answer is that the full downstream biology of NNMT inhibition isn't completely characterized. That's part of what makes it interesting — it sits at the intersection of NAD+ metabolism and epigenetic regulation, two active enough fields that the implications keep expanding. For researchers working on adipose tissue metabolism, NAD+ biology, or the epigenetics of metabolic disease, this is a tool that reaches all three at once.

NNMT (nicotinamide N-methyltransferase) is an enzyme that methylates nicotinamide, consuming both a NAD+ precursor and S-adenosylmethionine (the main cellular methyl donor). When NNMT is overactive — as in obese adipose tissue — it depletes the cell of nicotinamide and SAM. Inhibiting NNMT lets both accumulate, which feeds into NAD+-dependent pathways (sirtuins, PARP, etc.) and SAM-dependent methylation. The net effect in preclinical models is a shift in adipocyte metabolism toward greater energy expenditure.
Most published work is preclinical. Key papers from the Brenner and Bhatt labs at Cornell and elsewhere demonstrated NNMT inhibition reduces adiposity and improves metabolic markers in mouse models without caloric restriction. The mechanism is still being characterized — the NAD+ and SAM effects have independent downstream consequences that are actively being mapped. Human clinical data doesn't yet exist.
