What is PT-141?
PT-141 (bremelanotide) works in the brain, not in the blood vessels. That distinction matters. PDE5 inhibitors increase blood flow to peripheral tissue — a vascular, mechanical approach. PT-141 activates melanocortin-3 and melanocortin-4 receptors in the hypothalamus, affecting arousal and desire at the level of central nervous system signaling. Different problem, different mechanism.
CAS: 189691-06-3. FDA-approved as Vyleesi in 2019 for hypoactive sexual desire disorder — the first centrally-acting peptide approved for this indication.
Mechanism of Action
The melanocortin connection
PT-141 is a derivative of Melanotan II, but with narrower receptor selectivity. MT-II hits MC1R through MC5R and causes tanning, appetite changes, and sexual effects all at once. PT-141 was engineered to focus primarily on MC3R/MC4R in the hypothalamic arousal circuitry, with minimal melanogenic activity. It is the difference between a shotgun and a rifle.
Mechanism
MC3R and MC4R agonism in hypothalamic nuclei. These receptors are part of the melanocortin system — the same pathway that regulates appetite, energy balance, and pigmentation. The sexual arousal component involves melanocortin-dopamine crosstalk in reward circuitry. Notably, there is no nitric oxide or PDE5 involvement, which is why PT-141 works through a completely different mechanism than sildenafil or tadalafil.
