Melatonin is the primary circadian hormone, synthesized from serotonin in the pineal gland and secreted nocturnally in response to darkness. It signals through two GPCRs (MT1 and MT2) expressed throughout the brain and peripheral tissues, translating the light/dark cycle into a temporal signal that synchronizes circadian rhythms across the body. Beyond circadian biology, melatonin has documented antioxidant properties, immunomodulatory effects, and roles in seasonal reproductive biology in photoperiodic species. Each vial contains 10mg pharmaceutical-grade melatonin.
The antioxidant activity of melatonin is mechanistically unusual. Unlike classical antioxidants that donate a single electron, melatonin and its metabolites form a cascade of antioxidant molecules upon oxidation -- each metabolite retaining radical-scavenging capability. This "antioxidant cascade" provides sustained protection against oxidative stress. Melatonin also indirectly supports antioxidant defenses by stimulating expression of superoxide dismutase, glutathione peroxidase, and catalase. These properties have driven research interest in neuroprotection, mitochondrial biology, and aging contexts where oxidative stress is pathogenic.

Melatonin acts on MT1 and MT2 receptors in the suprachiasmatic nucleus (SCN, the master circadian clock) and in peripheral tissues to shift the phase of circadian oscillators. The phase-shifting effect is time-dependent: melatonin administered in the late afternoon/early evening advances the clock (earlier sleep onset), while morning melatonin delays it. MT1 receptor activation suppresses SCN neuronal firing (associated with the sleep-promoting effect), while MT2 activation appears more involved in phase-shifting. This phase-shifting property is the mechanistic basis for melatonin's effectiveness against jet lag and circadian disruption -- it pushes the clock in the desired direction.
Unlike most antioxidants that are consumed in a single electron-donation step, melatonin forms an antioxidant cascade: melatonin scavenges a radical, becoming AFMK (N1-acetyl-N2-formyl-5-methoxykynuramine), which is itself an antioxidant, generating AMK upon further oxidation, which retains some scavenging capacity. Each melatonin molecule thus contributes to multiple radical scavenging events through this metabolic chain. Melatonin also crosses all cellular compartments including mitochondria (where most cellular reactive oxygen species are generated), positioning it where antioxidant protection is most needed. This combination of cascade antioxidant chemistry and mitochondrial access makes it mechanistically distinct.
Melatonin has multiple documented anti-cancer mechanisms in cell and animal models: it inhibits cell proliferation through cell cycle arrest and apoptosis induction, reduces angiogenesis, enhances immune surveillance, and opposes the Warburg effect (the metabolic shift to aerobic glycolysis common in cancer). MT1 receptor signaling in some cancer cell types inhibits aromatase (relevant to estrogen-sensitive cancers), and melatonin modulates several oncogenic signaling pathways including NF-kB and PI3K/Akt. Epidemiological studies have suggested associations between disrupted circadian rhythms (night shift work) and increased cancer risk, connecting melatonin biology to cancer epidemiology. None of these mechanisms have been validated as cancer treatments in clinical trials.
