191 amino acids is the complete length of endogenous human growth hormone. Some lower-quality research preparations (sometimes called "192AA") contain an extra methionine residue at the N-terminus due to bacterial expression systems that don't cleave the initiator methionine correctly. This extra methionine can increase immunogenicity -- stimulating antibody formation against the GH -- which confounds long-term research. True 191AA GH matches the endogenous sequence exactly, reducing this problem and making it appropriate for extended studies.
GH's anabolic effects on muscle and bone are largely mediated indirectly through IGF-1. GH stimulates hepatic IGF-1 production, and circulating IGF-1 then binds IGF-1 receptors in muscle and bone to drive protein synthesis, satellite cell activation, and bone matrix production. GH also has direct effects on adipose tissue lipolysis that are independent of IGF-1. This dual mechanism -- direct fat metabolism effects plus indirect IGF-1-mediated anabolic effects -- is why GH replacement produces body composition changes (reduced fat, increased lean mass) in GH-deficient models.
Growth hormone is a protein, and freeze-thaw cycles can cause aggregation and denaturation that reduce biological activity without necessarily producing visible changes. Lyophilization (freeze-drying) removes water content and provides stability, but the reconstituted solution should be kept at 2-8 degrees C rather than frozen again. The bacteriostatic water used for reconstitution helps prevent microbial contamination during the multi-week storage period.
