Most satiety research focuses on GLP-1 receptors. Cagrilintide targets a different system entirely. It's a long-acting amylin receptor agonist — an acylated analog of human amylin (IAPP), the 37-amino-acid peptide co-secreted alongside insulin from pancreatic beta cells. The amylin receptor is a calcitonin receptor/RAMP complex, and its activation produces satiety signaling through brainstem circuits rather than the hypothalamic GLP-1 circuits that dominate metabolic peptide research. Novo Nordisk added an acyl chain to extend the half-life to approximately one week — the same fatty acid attachment strategy used with semaglutide.
Cagrilintide is being studied alongside semaglutide as CagriSema — a fixed-ratio once-weekly dual research protocol targeting two independent satiety pathways simultaneously. Preclinical and in-vitro COMBINE-style research has examined enhanced metabolic-pathway endpoints versus either component alone, which is the expected result when two distinct biological targets are engaged rather than simply doubling the dose on one receptor. The additive effect is the point.
For satiety neuroscience: amylin signaling converges on the area postrema and nucleus of the solitary tract — different brainstem nuclei than GLP-1. The RAMP protein combinations that form amylin receptor subtypes (RAMP1, RAMP2, RAMP3 each produce pharmacologically distinct complexes) add specificity that pure GLP-1 research doesn't capture. This is one of the more mechanistically interesting compounds in the satiety space precisely because it accesses a pathway that's often overlooked.
